Full Time: Mostly Monday through Friday 8am-4pm. Help as needed on all other shifts.
Pay Rate: $24.25 per hour

Position Summary:
As the Respite Care Home Coordinator, you’ll play a key role in supporting and enriching the lives of participants in our Respite Care Home (RCH). You’ll handle intake and referrals, guide new intakes and tours, manage participant records, and oversee vital documentation. Additionally, you'll train and support our staff in individualized care and record-keeping practices, ensuring a smooth and nurturing experience for our participants and their families.

What You’ll Do:

  • Coordinate and maintain participant records and documentation, ensuring accuracy and compliance.
  • Oversee individual plans for participants and guide staff training on support, behavioral needs, and safety.
  • Provide transportation to medical appointments and assist with hygiene, mobility, and daily care needs.
  • Build positive relationships with participants, families, staff, volunteers, and partner agencies.
  • Monitor the safety and well-being of all participants, family members, and visitors.
  • Administer medications accurately and document care activities through our Therap system.
  • Step in for the RCH Manager when necessary, ensuring continuity of care and support for the team.

Qualifications:

  • Education: Bachelor’s degree in social service or related field preferred; QDDP qualification is a plus.
  • Experience: Experience working with individuals with intellectual and/or developmental disabilities is preferred.
  • Skills: Strong communication, decision-making, and organizational skills. Proficient with Microsoft Office.
  • Physical Requirements: Must be able to lift up to 50 lbs, assist with wheelchairs, and drive a large van.
  • Certifications: Valid Driver’s License and ability to pass a Criminal Background Check, Family Care Safety Registry Check, and Motor Vehicle Report (MVR) Check.
