Community Living, Inc. is looking for a Vocational Case Manager to help individuals with intellectual and developmental disabilities discover meaningful employment opportunities. In this role, you'll work one-on-one with participants to identify their strengths, connect with local employers, and provide the support they need to succeed in the workplace.

One day you might be meeting with a local business to develop a new employment partnership, and the next you could be helping a participant prepare for an interview or coaching them through their first days on the job. Pay Rate: $23.25/hr
Schedule: FT 40 Hrs. Monday - Friday, mostly days and evenings depending on client availability

What You'll Do:

  • Partner with participants to identify employment goals and career interests.
  • Build relationships with local employers and develop job opportunities.
  • Conduct assessments to match participants with the right employment.
  • Assist participants with job searches, applications, and interviews.
  • Provide job coaching and on-site support to help participants succeed.
  • Monitor employment progress and adjust supports as needed.
  • Complete documentation, case management, and monthly reporting requirements.
  • Coordinate individual plans and maintain participant records in Therap.
  • Collaborate with families, employers, and interdisciplinary teams.
  • Ensure services meet all funding and compliance requirements.

What We're Looking For:

  • High school diploma or GED required.
  • Bachelor's degree in Social Services or a related field preferred.
  • Experience supporting individuals with intellectual and/or developmental disabilities is preferred.
  • Strong communication, organization, and time management skills.
  • Comfortable balancing multiple participants and priorities.
  • Ability to build positive relationships with employers and community partners.
  • Proficiency with Microsoft Office.
  • Valid driver's license and reliable transportation.
